To use the ASF Accredited Facility logo on your web site or other publications email: allyson@aaaasf.org or, click here to send an email, please include the following information:
1. Facilty Name or ID number
2. Type of publication or web site
3. The URL (example: www.yourwebsite.net) OR a proof (Word, Jpeg or PDF file type) of the publication for approval
Upon approval, logos will be emailed for approved use.

The American Association for Accreditation of Ambulatory Services Facilities (“AAAASF”) Symbol of Accreditation (the “Symbol”) is registered by AAAASF as a certification mark to permit owners/operators of facilities accredited by AAAASF to advise the public that their facilities meet the high standards necessary to be accredited by AAAASF.The following guidelines set out the requirements for the use by AAAASF Accredited facilities of the Symbol.

•  General Use - The Symbol is to be used only to indicate accreditation of the facility by AAAASF and for no other purpose. It may be used on the facility's practice stationery, in brochures relating to the facility, on facility practice forms, in advertising, on the facility's web site, and on business cards. The foregoing list is not intended to be all-inclusive, but illustrative of appropriate uses of the Symbol.

•  Integrity of Symbol - The Symbol may not be modified in any manner. No additional material or text may be placed immediately adjacent to or inside the Symbol, except for the trademark notice. If there is a question as to whether Symbol usage is correct, AAAASF approval should be obtained prior to production. Such approval will be limited to placement and usage of the Symbol and will not constitute an approval of the user's advertising or promotional materials.

•  Forms of Notices - All users of the Symbol are strongly encouraged to reproduce the Symbol using the same position, type style (Novarese Book) and weight used by AAAASF.

•  Placement of Symbol - All users of the Symbol must take steps to ensure that the Symbol is not placed on any item in such a manner as to give the appearance that the Symbol is owned or controlled by any entity other than AAAASF.

•  Statement - The following statement is recommended for all uses where possible:

This Symbol of Accreditation designates facilities which are accredited by the American Association for Accreditation of Ambulatory Surgery Facilities. Facilities which display this Symbol are dedicated to the highest standards of patient care.

•  Advertisements - The Symbol must not dominate any advertisement in which it is used, or in any way create the impression that the advertisement is sponsored or paid for by AAAASF.
