As an accredited facility, you are on the leading edge of the compliance process. You have already met the standards for accreditation and are just one step away from completing the state requirements. The State of New York requires AAAASF to poll our current facilities for their organizational structure. Please click here for the Polling Form and fax back the completd form to the AAAASF office at 1-847-775-1985. This information will be shared with the New York Department of Health and kept in your confidential facility file at the AAAASF office. Another component of compliance required by the State of New York is the reporting of adverse events beginning January 14, 2008 . The following are links to the New York Guidelines for Reporting Adverse Events and the reporting form for your convenience: http://www.nyhealth.gov/forms/doh-4431.pdf and http://www.nyhealth.gov/forms/instructions/doh-4431_instrucions.pdf . AAAASF is working with the State to develop a way for AAAASF to pass through the adverse event reporting from the facility peer review report submitted to AAAASF bi-annually. In the future, AAAASF facilities could have the added value of only having to report once and the information would be shared with the New York Department of Health to assist you in meeting this requirement.
